BHU 2013 PMT Offline Application form:

BHU 2013 MBBS/ BDS/ BAMS/ B.Pharma Application Form can be obtained by following methods:

BHU 2013 APPLICATION FORM BY CASH : BHU 2013 Medical Exam Application form can be brought through the payment of Rs. 1200/- for GEN/OBC and Rs. 800/- for SC/ST candidates from the following branches of Bank of Baroda.

VARANASI – (i) BHU – S S Hospital Branch, (ii) Ram Katora Branch, (iii) Orderly Bazar Branch
DELHI – (i) Parliament Street Branch, (ii) Nehru Place Branch, (iii) Chowk Shakti Nagar Branch
KOLKATA – College Street Branch

HYDERABAD – Main Branch, ABIDS Circle
JAIPUR – DCM Branch
BHOPAL – TT Nagar Branch
PATNA – Sri Krishnapuri

BHU 2013 APPLICATION FORM BY POST: BHU 2013 Medical Exam Application form can also be brought from the

Office of the Director (PMT-Cell),

Institute of Medical Sciences,

Banaras Hindu University,

Varanasi-221 005

by sending a written request through Registered Post along with Crossed Demand Draft/Bankers Cheque (MICR only) of Rs. 1300/- for General/OBC candidates and Rs. 900 for SC/ST candidates of a Nationalized Bank, drawn in favour of “The Director, Institute of Medical Sciences, BHU” payable at Varanasi. Application form will be sent by Speed / Registered Post.

NOTE: BHU Institute will not be responsible for any loss or postal delay. Candidates are advice to attach the request for application form must include the name and address of the candidate, and full particulars of the enclosed DD/BC (write candidate’s name and address on the back of DD). Two self-addressed, preferably typed, white paper slips of 10 cm ´ 6 cm size must also accompany it. The requisition for BHU PMT Application Form should be superscribed “Request for Application Form for PMT-2013, BHU”.

Last date of requisition for BHU 2013 PMT form by post is 21.02.2013.

I visited Bank of Baroda,College St.Br. Kolkata and requests for the form of BHU MBBS entrance test. In reply they said you will get it from the website of BHU. Pl.advise from where I will get the same?
